Between £40,000 and £45,000 dependent on experience and qualifications
The successful applicant will play a lead role in medicines management, optimising prescribing and improving quality.
The post holder will be an experienced pharmacist who will work to develop, manage and mentor the PCN clinical pharmacist team, currently comprising of three clinical pharmacists and a pharmacy technician.
The post holder will undertake structured medication reviews to proactively manage patients with complex polypharmacy, especially for older people and those in care homes. The post holder will implement prescribing decisions and request appropriate monitoring investigations.
The post holder will work closely to support the wider practice teams to support safe and effective prescribing, whilst conducting F2F clinics.
